MEDICARE READMISSIONS CERTIFICATE PROGRAM



The Centers for Medicare and Medicaid Services' recent changes in reporting 30-Day readmissions and consideration of payment initiatives for some diagnoses has left many organizations seeking strategies to improve their care transition performance. The Obama Administration's requested policy changes in health care budget proposals and health care reform include reducing payment for 30-Day readmissions as both a cost saving and quality inducement tool. There are a variety of important dimensions and considerations - consumer, service, payment, and data systems. At the same time, there are a variety of innovations and technologies that could help address the challenge of managing those transitions and preventing readmissions.

The National Medicare Readmissions Summit is the leading forum on Medicare 30-Day Readmission policy and technology-enabled strategies for transitions of care. At the Summit, participants will:
