MEDICARE READMISSIONS CERTIFICATE PROGRAM



The Centers for Medicare and Medicaid Services' announcement that they are considering both reporting and payment initiatives for 30-day readmissions for the same diagnosis has left many organizations seeking strategies to improve their care transition performance. There are a variety of important dimensions and considerations -- consumer, service, payment, and data systems. At the same time, there are a variety of innovations and technologies that could help address the challenge of managing those transitions and preventing readmissions.

The National Medicare Readmissions Summit is the leading forum on the new Medicare 30 Day Readmissions rules and technology-enabled strategies for transitions of care. At the Summit, participants will:

  • Understand the potential impact of new Medicare 30-Day Readmission rules on reporting requirements and financial performance of hospitals
  • Understand the larger context of CMS strategies to lower cost and shift care to less resource intensive settings
  • Learn about successful strategies for managing readmissions and transitions of care
  • Learn about specific technology enabled innovations central to reducing readmissions, improving clinical quality and reducing cost

The Summit brings together experts in payment policy, care delivery, operations and technology and innovation to explore the many issues and opportunities for improving 30-day readmissions performance for patients, providers and policy-makers.

This Summit will break new ground by permitting registrants to attend the complete conference experience either onsite or online through both real-time and archived access to all Summit presentations. For many reasons (including the international economic crisis, challenging air travel, the proliferation of higher bandwidth and YouTube, and the adoption of online training by the health sector) we believe that now is the time to be more innovative in approaches to sharing knowledge. Accordingly, the Summit will be a hybrid conference, Internet event and training tool.
